The quest bugged for me. There was no extra action button to use the vial to stop her from regenerating. I ran away and dropped the quest. Picking it up again didn’t fix the issue. The button (as seen in the video on this page) did appear for a second or so before the cutscene, but was gone afterwards. I’m running with no addons, so that wasn’t the issue. I just killed her without the vial, but the fight was very drawn out. I kept saying ...things... in response to the NPC telling me to use the vial. =) I reported the bug in-game, so hopefully it gets fixed before release.

The Extra Action Button did not appear for me until I died. Ran back, and killed her. Didn't even have to use the vial.

Quest Name:	Start and description
1 The Grove of Terror	Start quest from Sire Denathrius 53.4 64 go to see The Fearstalker 50.7 73.8
2 Dread Priming	Go to crypt after door will be open press on Houndmaster Loksey and speak with Sinreader Nicola . Take Houndmaster Loksey thru the path demoralized by garcons. after all go to Flockmaster Sergio 51.2 74.3
3 Alpha Bat	Go to Stake 52.5 76.4 and bound Houndmaster Loksey in few sec Duskterror will come get it to 1% and will be captured.
4 King of the Hill	Go to The Fearstalker 48 70
5 Let the Hunt Begin	First press on Houndmaster Loksey then use Hollowed Dredbat Fang
6 The Penitent Hunt	Take this quest together with Devour This. Speak with The Fearstalker to start. Kill Truulax 46.5 65.5 next one is Yoshai the Merciless just press on him and last is Crimelord Tiana same press on him.
7 Devour This	Kill 12 mobs
8 The Accuser	Go to 51.4 59.8 and press Midnight Rose ( watch your head xD ) then speak with The Accuser you will learn new spell Door of Shadows use it to jump thru Wall of Sin.
9 A Reflection of Truth	Go through the mirror then speak with The Accuser and animation will start. After all Protect Her killing incoming mobs when will get to 100% go through the mirror to escape.
10 Dredhollow	Use Door of Shadows to jump thru Wall of Sin ( that barrrier can even kill you O_o ) and go to The Accuser 43.2 73.8
11 Rebel Reinforcements	Do this quest together with Breaking the Hopebreakers and They Won't Know What Hit Them. You need to recruit Watchmaster Boromod He is in Crypt enter 42.9 75.7 and Tactician Sakaa 40 73.2
12 Breaking the Hopebreakers	Kill 3 mobs Hopebreaker Marku 43.5 79.4 Hopebreaker Mursa 41.4 78.8 and Hopebreaker Vilstav 40.3 74.3 before figh with them use Concentrated Anima Vial.
13 They Won't Know What Hit Them	Pick up 8 Rigged Anima Container. after finish this 3 quest move to 39.8 81
14 The Fearstalker	First speak with The Accuser will be animation. If The Fearstalker will no show up after animation just wait few sec like i had xD. Kill The Fearstalker and pick up Medallion of Dread then move to The Accuser 43.2 74 That will finish this storyline.

The Fearstalker: I grow tired of this farce. Show yourselves, and I will grant you the mercy of a swift end.

<You signal to the Accuser that you are ready.>

<A short cinematic starts.>
<The Fearstalker and two Hopebreakers ride into Dredhollow, triggering the anima trap you laid around the village. The trap goes off and attracts an elite Worldeater Colossus, which quickly kills the Hopebreakers and severely wounds the Fearstalker. She gains some distance before she casts a spell that kills the colossus. She now stands before you, the Accuser, and General Draven at the Fearstalker's Lodge beyond Dredhollow.>
<Cinematic ends.>

<The three of you engage the Fearstalker. She occasionally tries to siphon anima from the downed Colossus, but is interrupted by the Mites attracted to the concentrated anima thrown on her.>
The Fearstalker: We will snuff out this resistance. In the Master's name!
The Accuser: She is trying to siphon the anima! Use the vial, mortal!
<Finally, the Fearstalker falls. From her corpse, you claim the Medallion of Dread, bestowed upon her by The Sire himself, allowing her to exert partial dominion over Revendreth.>
The Fearstalker: The hunter becomes... the hunted.
General Draven: A decisive victory. We will meet you outside Dredhollow, mortal.

This quest was bugged when I did it...couldn't put a dent in her, the vial didn't do anything and she stayed at full health. On my 6th try someone else was there and she dropped quickly.

What a terrible cut-scene. They really need to update the graphics in this game!

On my Rogue, Draven never attacked at all - just stood there while The Accuser did hardly any damage at all. My Warlock was able to kill The Fearstalker without help (very long fight for me) but my rogue was helpless. All addons disabled - made no difference.

Update - 2021/10/22, nearly 9 months later - exact same thing happened this time on my feral druid. Long ago, Blizz used to fix things like this - but no more. In both cases the Accuser did no damage at all either (although at least she was trying unlike Draven). No addons, reloaded, logged out and back in. I managed to get Draven attacking a few times but he must have got tired as he just assumed his inactive stance. I've done this quest on over 15 toons and most times it was easy, just interrupt the anima drain. Oh yeah, many years ago we used to have in-game Game Masters who you could summon, they would often intervene and fix things in real-time.

This quest must be bugged even further.
After I completed They Won't Know What Hit Them and Breaking the Hopebreakers Nothing happened..
I clicked The Accuser, to talk to her and it just pops up the empty dialog box with "Are you ready?"
with nothing to acknowledge it.

I'm stuck!

If you are caster without a pet you are screwed. The fight requires a good deal of movement. The mob teleports
which interrupts casts which when combined with the seconds it takes to hit the extra action button to stop him
from healing makes the fight all but unwinnable without some help.

I and added some leech (Siphoning-Weapon, Fortified Leech-Cloak) to my Bal Druid and did a good bit better,
on round 2. On round 3 I pulled him towards the front of the building and it seemed to separate him from his
healing which shortened the process immensely. If you go too far he will break off and go back to his original
spot but he doesn't reset his HP's.

Killed her, got no medallion. Waited for her to respawn, than got it.

I had the action button for the vial, but the Accuser does no damage and Draven is just a spectator. Not helpful, guys. I solved this by getting her health down as much as possible before dying and then rejoining the fight as quickly as possible before she starts regenerating. I used the vial whenever it came off cooldown for its "stun" effect. Took a couple of cycles of dying/resurrecting to finally kill her.

Quest was bugged for me today. I had the vial buff but no button to push when she channeled the spell that healed her. /reload did not work for me so I tried a macro instead which worked thankfully.

Fight seemed to go on and on and Vial didn't stop Fearstalker from healing so I ran away until I dropped agro.
I went back and picked her up again and pulled her to top of stairs there and I was able to burn her down without ever using Vial.
Get her away from that regen thing.

I completed the two preceding quests (Breaking the Hopebreakers and They Won't Know What Hit Them) and The Accuser had no follow up quest. Ran to the entrance of Dreadhollow and grabbed a quest from General Draven to recruit 2 of his people. Once I turned in that quest I was prompted with The Fearstalker.

Speak to the Accuser to trigger the trap, then kill the Fearstalker and collect the Medallion of Dread.

Description

The trap has been set and the Fearstalker has walked right in. Just as I expected, her pride has blinded her to the obvious. Have that vial I gave you earlier ready. It may prove useful in the fight ahead. When you are ready, we can spring the trap.
