Ok, This is related in some way to the possibility of upgrading your orange cloack Ashjra'kamas, Shroud of Resolve

All this 3 things, Vessel of Horrific Visions , Corrupted Mementos , Coalescing Visions , are all connected into this upgrading process.

This is the full explanation

As you may know, Horrific Visions are some kind of scenarios to play solo or in group. All players will explore the same Horrific Vision throughout the week, either Orgrimmar or Stormwind. It will then alternate between the two cities every week.
You can queue for the Horrific Vision by interacting with the gateway located in the Chamber of Heart next to MOTHER.

Wrathion will give you quest to do/complete inside those scenarios. Doing this will reward you with items to upgrade your orange cloack.
This is one of those quest Reconstructing "The Curse of Stone" and ask you to retrieve some of this Torn Page of "The Curse of Stone"
And where those pages comes from? They come from the chest rewards and from those you will be required to do/kill a Main Objective - Tainted Areas or to do/kill one or more Bonus Objectives - Corrupted and Lost Areas.
To queue for one of those scenarios you need to have one Vessel of Horrific Visions in your bags and will be consumed once you zone in.

Two basic ways to get Vessel of Horrific Visions, first way is been lucky because those can drop from inside the chest/cache that you will have as reward for complete the Main Assault every week. Second way is to buy em from Wrathion, every Vessel of Horrific Visions cost 10.000 Coalescing Visions

And where and how to earn those Coalescing Visions?
Mainly with Assaults. As you many know, there are one Main/Major Assault every week and two Minor Assaults every week.
Completing the major assault will reward you 10.000 Coalescing Visions, and completing every minor assault will reward you 5.500 Coalescing Visions
Just doing this you will earn over 30k every week so 3 attempts/scenarios.
Also there is a portal in the Main weekly assault map that allows you to do once a day a Mini-Vision scenario, doing this will award you with 2.000 Coalescing Visions the first weekly completion and 1.000 Coalescing Visions the next 6. So 8k per week, add to this the excess from 30k from Assaults and you will have one more try to scenarios every week.

Of course, you have daily quests in assaults but those are 3 per day and reward 50 Coalescing Visions each one, so.... only worth the efford if you are close to buy one more token.
IMP Note about Coalescing Visions, those cap at 25.000, as soon as you have over 10k, use em to buy one Vessel of Horrific Visions, or you risk to do/complete something and lose the ones you earn if those overpass the cap limit.

And how about the Corrupted Mementos ?
You will earn some of those while doing the same things you do to earn Coalescing Visions, but not a lot.
The most important source is from doing the scenarios, and you will use this to research upgrades into Titanic research archives so you will earn "talents" that will help you to do Horrific Vision Scenarios easier.

Another way to get Coalescing Visions is from the N'Zoth corruption mobs.

In short just go into a horrific vision and kill the boss again.
