The path is up on the right as you face the instance entrance. Up where the little huts are then through a stonehengy arch.

This quest is not seeming to trigger properly for me. I've gone up the path, I see Yrel and Velen (and also Grommash and Ner'zhul) and a swirling purple pool but none of the NPCs are interactive and no events trigger. I've tried everything I can think of: Dropping the quest and picking it back up, making sure I'm not grouped, walking up the path rather than riding up it mounted, making sure all other quests in the area are completed and turned in before re-picking up this one. Nothing seems to make a difference. Anyone run into this and know how to solve it?

Update: What worked for me to fix it was to group with some friends who were doing the quest. It triggered properly for them and counted for me. I was never able to fix this on my own.

easy way to find Ner'zhul:

Starting from the square stone pit where you slay the Essence of Darkness
go NW about 40 yards.

Standing (at 30.7, 38.7) between the two large posts with blue flags and white quarter moon icons,

look SW uphill and the arch you want is the leftmost which you can see from between the posts.
(arch is at 28.5, 41.1 )

Follow the path up to top of the rise and the quest NPCs. You need to interact with the friendly NPCs
and wait for some dialog.

The NPC you need to kill for credit is Arkeddon.

NOTE: at this time, quest seems to work fine solo (lvl 92 BMH)

Several people have mentioned this quest not working correctly. The most likely reason is that they're not beginning it at the right time. You have to start the quest right when the lion in the MGM logo roars, that way the lyrics will line up properly with the action.

Yrel and Velen are inside the fortress. Easy entrance is at 31,37. Take right hand path at 31,39 - trail demarcated by torches. Through pilllars at 29,41. Up torch lit trail to yrel and Velen at 31,43.

When you reach the Terrace of the Stars:

Yrel says: Are you ready, hero? It is just up this way.
Yrel says: Over here, hero, before they see us! Listen - they're talking about the Dark Star.
Ner'zhul says: Behold my power, Warchief Hellscream. With my clan at your side, you will be unstoppable.
Arkeddon yells: The void...HUNGERS!
Grommash Hellscream says: Well done, Ner'zhul. We attack Karabor soon. Summon forth the Dark Star.
Ner'zhul says: I will begin my preparations at once.
Yrel says: What is that monstrosity?
Prophet Velen says: It is but a fraction of the Dark Star's Power.
Yrel says: We'll see about that!

Yrel says: Samaara!
Yrel says: No. No!
Samaara says: Yrel, do not be sad. I did not choose this fate, but you...you can still choose.

How to reset this quest:

Abandon the quest, run back to the entrance of the Fortress, speak to Velen and re-accept the quest. When you run back to the starting point on the hill Yrel will be there and the quest will be reset.

Another plus to this is you can turn in the other quests you previously completed.

Oh COME ON! Velen is RIGHT THERE, no more then 10 feet away and he doesn't cast a single Holy Light or Greater Heal or even a Renew? WTF Dude, she wasn't even dead yet and you did nothing to save her. Seriously?

I completed this quest and abandoned it before turning it in because Yrel wasn't spawning. I've abandoned and re-accepted this quest several times now and every time Velen is just standing by Samaara and I cannot complete the quest now.

This quest line made no sense at all. I thought the whole purpose of opening this place up was so Dranei could attack it, didn't see that happen at all.

Do you have to go through the Terrace of the Stars portal or what to get here? I get logged out when I hit the portal and I don't much like the idea of going around.

For those with issues where Yrel and Velen are just standing there. After a few dozen times, found a way to get the quest to properly respawn. I found the main reason for the problem is I had flight, and was skipping around a bit to other quests, missed the main trigger. No matter what I did, I could not get the quest to trigger again, dropping several times, etc.
What I had to do was fly a long way from the entrance, way way out. If you think you're far enough, fly out a bit further. Then drop the quest.
Fly back to the main entrance and pick up the quest from Velen, then run on foot all the way the archway, no flying. run through the archway, up the hill, and Yrel should be standing part way up waiting for you, and away ya go.

If you're bugged, realm hop. Be sure you're in a party not a raid. It will work.

When I completed protecting her, it showed a gold question mark at her location, but she did not give me the option to turn it in. I ran away from her location (back to the cave) then ran back to her and she finally let me turn in the quest.

If the quest won't start right, abandon it & pick it up again.
This time, go right to Yrel at 29, 42. She will start talking and run over to where the quest continues.

This feels like a problem caused by flying past Yrel without stopping to let her run over.

This wasn't triggering for me, even after reading everyone's comments here. So here's what ended up working:
1. Abandon quest where the turn-in is, watch everyone disappear.
2. Fly into the portal to Shadowmoon Burial Grounds.
3. Leave the instance.
4. Return to Velen without using a mount (I used Aviana's Feather to get out of the pit).
5. Walk back up the path without mounting.
6. The event trigger as Yrel met me halfway up the path.

I'm sure not all of those steps were needed but it did work for me. I tried pretty much everything listed here to no avail before stumbling upon this solution.

Bugged out for me. Seems it doesnt like flying mounts. I tried resetting a few times but nothing worked until I did the following:

Dropped Quest
Hearthed to garrison
Rode back (Not fly) to Velen
Ran back up the hill.
Quest finally worked.

To reset the quest, I abandoned it and used my Garrison Hearthstone to teleport back to my fort. After that, I travelled back to the steps leading to Anguish Fortress, and accepted the quest again, continuing as normal.

My flying mount (Snowy Gryphon) didn't seem to affect the quest again, but others have reported that a flying mount will cause it to bug out again, so use at your own peril, I suppose!

Yrel and I have located Warlord Ner'zhul. He is meeting with Grommash Hellscream atop the highest altar overlooking Anguish Fortress.

Meet us there. We must stop him.
