Defeating her in a duel rewards the Headmaster / Headmistress title (no achievement).
You must defeat the living version of her by using the Key to Nar'thalas Academy to get the title.

To get to this NPC, you will need the Key to Nar'thalas Academy, only way to get them are through the Archaeology quest Academic Exploration. Using them in the Nar'thalas Academy will send you back in time, where they were alive.

So I was a bit confused as to this title, Headmistress/Headmaster. You do not aquire it through an achievement, you'll just earn it by defeating her in a duel however. You are not able to just duel Elya Azuremoon as soon as you enter the place, you will need to "study", so to speak, first.
How I did it was speaking to Sythorne, and just did all 3 of the Kobold scenarios. First one is just a standard mob with 3.12M health, Kobold Country, 2nd one will be a bit harder with 9.35M health, hence recommended for 3 players, A Guide to Kobold Societal Mores, while the last one will be recommended for 10 players and 41.57M health, Before the Candle: The Rise and Fall of the Great Kobold Empire. I did them alone however, it just took quite a while as I was a Holy Paladin, but none of the abilities done by any of the mobs were actually very hurtful. The same with Elya Azuremoon, she is recommended for 10 players, she doesn't actually do anything hurtful either though, I do recommend actually dodging whatever you can dodge however. Multiple hits on you might send you low on health. She has a health value of 39.91M herself, which is less than the 10 recommended Kobold Drawing.

Soloed as an arcane mage. Strict Discipline is mostly absorbed by either Mirror Image or Prismatic Barrier, Counterspell everything you can, dodge behind pillars during Arcane Missiles+Slipstream. She is affected by Slow and Frost Nova. Kite, hump pillars and dps her down. Takes a bit but it works.

So.... i got my Key. And i started the Fight. Then at the end my Follower http://www.wowhead.com/champion=986 just ate her. For a few seconds there where a living neutral pile of Meat. Now she is just gone and wont respawn XD Well. I got my Title ^^

Beat her as an ilvl 900 shadow priest by putting dots on her, used mind blast, but mostly by running around like a chicken with its head cut off.

If you ask this NPC to duel you and replies with: "Not now. Spend a little bit more time interacting with other instructors, and I might consider it" that means that you have to do all the training with Instructor Sythrone by the bookcase and Instructor Nidriel by the stairs before you reach Sythrone. Make sure you complete all 3 Koboolds, all 3 Sea Skrogs and all 3 Blue dragonflight challenges from Sythorne. Likewise with Nidrial, need to complete all levels of Freshman, Sophmore, Junior and Senior rune drawings for each spell class (Arcane, Fire and Frost). Click each spell class to see and to choose the levels.
