This is obtained by clicking the Pirate Pepe in Spires of Arak, which is also a step for the I Found Pepe! achievement.

This item isn't really an "on use" item as it is learned when used. When you right click the item, you might be sad thinking it is a one-time-use item, but it learns the Pirate Pepe spell to your character, so when you summon Pepe with your bird whistle, he'll sometimes be a pirate. If you learn the other 3 hats, he can randomly be one of the four, so if you prefer a certain Pepe, just don't learn the other hats!

This can be found on the west coast of Spires of Arak east of Pinchwhistle in a set of semi-submerged ruins. There is also an elixir of shadowsight in this location.

Does anyone know if the achievement/random hats on Pepe will work account-wide once completed on one character, or if you should complete the search for the four Pepes with each character you sometimes summon Pepe for?

These items "teach" you the chance to summon a costumed pepe in place of the regular skin in future after they have been used. You will not 'waste' them by using them right away without your friends and guildies flocked around you.

This is all confusing to me, because my first (and so far last) interaction with Pepe the bird is when i found Pepe in Spire's of Arak at 54.1, 83.6 as DanJack so kindly and helpfully mentioned, but i didn't know about Pepe's existence at the time, and just noticed i could right click on him, so i did! And when i did, i got this item. And i right clicked on this item, and suddenly Pepe was on my head wearing a pirate hat! However, the item disappeared from my bags. And didn't become a new toy in my toy box, or a pet in my battle pet collection, either. There is no item named "bird whistle", which someone mentioned is what's apparently supposed to happen, how you summon him more than just once? So either i'm missing something, or it messed up for me, or it really is just a one time use, only summons Pepe one time, unless you have obtained Pepe in some other way and looting this item does indeed just add the chance of him wearing a pirate hat.

But for me, all it did was summon him on top of my head for one hour and now he's gone never to be seen again!

*UPDATE*

After much searching for answers, finally i was able to figure it out. So for those of you who come here clueless like me, here's the deal:
You only get an item to actually summon Pepe after you complete the achievement Trans-Dimensional Bird Whistle. Until then, the only way to get Pepe on your head is by right clicking on him where he lives, in a tree in your Garrison. At everything level 3 Garrison, Alliance (which is the only place i know where Pepe is), Pepe lives on a low hanging branch on the tree between your Barracks and Trading Post, right on the edge of the pathway.

While you are here, get the Spires - Treasure 048 - Elixir of Shadow Sight 06

The item is in the water. Swim towards the large piece of wreckage in the water and swim to the end farthest away from the land (53.13, 84.53). It is under the wreckage tucked under the wreckage. You will not be able to see if from the surface.
