((shrugs)) I'm Fury specced and generally don't have a hard time keeping aggro. I stay in Defensive Stance when tanking, using Sunder Armor on solo targets and Thunderclap/Cleave on groups, with my finger on the Taunt hotkey in an emergency. Using Revenge whenever it comes up helps too. It also helps to remind party members that when they take the aggro from you to *stop* damaging those targets for a second or two to allow your taunt+follow-up attacks to take them back, especially if you're the lowest level in the group.

While you may be having issues as far as your gear being replaced, generally that should be the same for the players you're grouped with...so I wouldn't say that's a definitive cause.
You can lose agro for *many* reasons. Here's a few questions that hopefully will help us establish what the underlying problem is:

Anyone can pull agro off any tank...but they're not supposed to.

Tanking is not just up to the warrior, everyone needs to understand the warrior and play alongside the warrior. Not as if they were soloing.
Just tell the group to give you 2 seconds after every pull to initiate aggro.

You can start out with a taunt so they don't pull initial aggro. If I know who I am playing with I know a few people in my guild like to start out hitting hard. Thats why I taunt first.
Some mobs (like casters) I DW to do more dps giving me more threat and just spam devastate and HS. Since they only cast spells, a shield doesn't matter much except if you want to spell reflect a lot.
I DW on the casters in ZA on the way to the bird boss. I would keep a shield on for the casters in Mech that cast the pyroblast type spell the I can reflect back for like 2 or 3k.
You just gotta adapt to the situation.

Honestly, in normals and on some mobs on heroics, I don't even get a sunder up. I will use shield slamm (17 rage) and Revenge(2 rage) and run out of rage because the mobs won't hit me, and by the time I do I can use shield slam again so I use it again.

im fury but i MT sometimes in dungeons (not heroics im 66) and im doing well (if the party using threat meter or knowing when to stop dpsing)
i mark, i put 3 sunders on main target, demo, thunderclap, and switching the targets while applying 1 sunder on each. and it goes fine, till some genius starts nuking another target and call for help? so what i do? i let him die and so he knows that he done wrong,, and most of them start letting the tank get aggro b4 they nuke coz they know u will let them die if they behave like that.. and in some cases they leave the party lol...
Anyways what i think of a tank is this;
1- maintain aggro
2- protect healer at all costs
3- other party memebers should take care of themselves and tell the tank once they get aggro
4- let them get a threat meter such as omen will make the party alot more effective.
