if you were in the perfect raid setup with every available buff fighting a patchwerk fight and hitting every correct button at every correct moment that is what you would hit.
this is the limitation of simulation. essentially simcraft is saying what is the absolute maximum possible for your gear in a tank and spank, single target affair.
turn all the buffs off and you'll get an idea of what you should be able to hit on a target dummy if you press all the right buttons at the right times - this is the number you should see as an achievable goal and strive for.

yeah also, heroics are about as far from a measurable sim-able situation as it's possible to get.
the assumption is that bosses are the things that drop the purples we want so we discuss and plan for that aim on sites like this. the further assumption is that, since you can't simulate 'real' boss fight conditions (blizzard rarely likes us to stand and nuke in fights), the best thing you can do is plan around ideal situations and make decisions based on that.
heroics are about as far from this idea as it's possible to get but making decisions (stat weights etc) based on them will still afford you some good results in heroics.
