My Healbot keeps track of all of that... WS Debuffs, PoM tracking, and Grace. I just keep Penance on my hotbar and watch it for the cooldown to use it again. Your priorities as a disco priest should be PoM, Bubble, Penance, Flash Heal, Renew. FH should only be used if your Penance is on CD. There's otherwise no good excuse to use it over Penance.

I use a combination of Power Auras and Grid. Set the icon in the middle of the gred frame to PW:S and WS debuff. That will tell you if you can shield them or not! its very handy

You said you rolled disc, but that doesn't tell us what you were healing. A discipline tank should be healing one target, a tank. Raid heal if need be, shield someone low on hp, but for the most part, you focus on keeping a tank alive, we're similar to pallies in that aspect.
As far as your UI goes, I used to use Healbot, and it works to a degree, but once you start getting into 25+, it gets too bulky for my tastes. It took me a while to switch to grid+clique+grid status hots, but after that, I'm much happier. Within that small grid box, I track aggro, grace, PoM, renew, weakened soul, and magic and disease debuffs...along with the hp of the target of course. Not to mention, grid naturally lets me know the stack of grace, and when PoM and renew are about to expire (based on the color). Furthermore, I was able to use the exact key bindings as I had in Healbot.
As far as your CDs go, the big one to worry about is Penance, and eventually you just learn when Penance is up, it's a short CD so I rarely have to look to see it, I just get a feel of when it'll be ready...not to mention if I'm healing all out, I tend to do Penance > FH > FH and just repeat the cycle. Your other CDs, PI and PS, have CDs w/ a decent length, but you also don't have to cast them asap after thier CDs expire. Use them when needed.

IMO, Disc is kinda class that suits to all roles.It only needs the player controlling it has to coalesce with disc healing.
Forexample, if you're assigned on MT, and if MT is not taking High damage, When you realise raid needs some help, You can just PW:S + POM tank and give the raid one or two POH.I know it has a long cast time and consumes mana hard, But it saves a lot from dying when used in right time.
Another example, You can save your POM cd just before XT-002 Deconstructor's aoe dmg, and can cast it to a raid member as he does that.By this way you help the raid healers a lot.
Disc Priests in raid, Should not stand still and only protect a single target, if you're a disc, you always have to be awake, and you should help all other healers when needed.Because you are a damage stopper.
About the other subject of this page, I used to use some stuff for watching the CD's.But when you %100 coalesce with disc, you just know when your CD's are over.You don't even have to watch for them.As Aldones said; "I just get a feel of when it'll be ready".
As we all used to be, some may not know abbreviations used here.
PW:S = Power Word Shield
POM = Prayer of Mending
POH = Prayer of Healing
FH = Flash Heal

@ Healbot and PW:S, Mine shows a shield icon on my target in the frame when I cast the shield and it stays there until Weakened Soul goes away. I thought at first the shield was up the whole time, but of course it isn't. If I watch closely, I can see the tank lose the shield almost immediately and when WS fades away, so does the shield icon on his frame. It also has a 10 sec countdown. I assumed this was normal and came on the "stock" set-up. I don't know much about addons and don't think I modified mine to make this happen.
Just tested this;
- When u use PW:S the weakened soul debuff is shown by the PW:S icon with a small + sign in the top left corner.
- If Weakened soul runs out and PW:S is still on the target the + sign in the top left will not be present
- If Weakend Soul has expired and the shield isn't on the target then no icon will appear.
Hope this helps clear some things up for you.

@mojo -- Most excellent. I find it essential to see both the buff (PWS) and debuff (WS) on my targets' raid frames. Sounds like Healbot has a nice system for this (as you described). In Grid I use two separate corner icons (see my earlier post for screenshots).

They're a little buggy if you're lagging and messing with GCDs, but I found an icon with the time until I can cast it a whole lot more readable than a spiral or even a bar. Skin them with
ButtonFacade
and you can start to customize them.
Everything else you've said requires a lot of tweaking with Grid (or oUF_Freebgrid). Any major unitframe addon has some sort of aura tracking. Telling Grid to track who has your Renews, who still has a Weakened Soul debuff, who has incoming heals is a lot of work, but it will give you that information in a glance.
I layered my PW:S over the WS debuff, so I know when that has expired. My Renews are tracked and turn to little red squares when they're about to expire. Prayer of Mending isn't tracked. That's a waste of space. I see who has agro, and who has incoming heals. I have my CooldownButtons right under my cast bar, and as the icons disappear, I know they're able to be used again. Big icons are on people that have debuffs, and ones that I can dispel are displayed over ones that I can't.
I could use something like Power Auras classic, but that stuff doesn't fit with anything on my UI.
