Looking back on Ulduar and Obsidian Sanctum, Blizzard has decided to implement the idea of "Hard Modes" in order to challenge raiders, baiting them with savory rewards of all sorts, such as achievements and better loot.
This thread is dedicated to inventing your OWN hard modes. You can use any boss (or heck, any mob worthy of a "hard mode") that doesn't already have a hard mode. You can also modify or add a hard mode of a boss that has one if you wish.
For example:
Patchwerk (Hard Mode)
Activated by: Defeating all the trash mobs before Patchwerk without dying and within 15 minutes.
Health: +25%
Damage: +10%, but increases by 10% for every 10% health the boss loses.
Hard Mode abilities: Crushing Blow: Smashes the ground, instantly dealing 10,000 physical to all melee (before damage modification) and 4,000 to any target over 10 yards away.
Rewards: 1 bonus ilvl 213.

Mr Smite - Hard Mode
Trigger
: Pull Mr Smite, and then kite him all the way around the first level of the ship, off the ship, and down to his weapon cache before his second stun.
Effects
: When Mr Smite uses his second stun, he will yell "Alright, men! It's time to REALLY improvise!" and any of the remaining trash alive as well as Mr Smite gain a 20% increase to damage. You will have to deal with all of the trash (and Cookie) during the pull.
Reward: Mr Smite drops one of each item on his loot table, and you get the achievement .

Kite Patchwerk across Frogger, while killing the adds in Grobb's room, then defeat Patchwerk while Grobb is engaged. Now that's a real hardmode.

Noth (Hard Mode)
Activated by: Killing Gothik at least 20 minutes before Noth.
Health: +50%.
Damage: +25%.
Abilities: Teleports more frequently.
Summons 2 extra adds during the ground phase with each summon.
Also summons 2 extra adds during the teleport phase.
Adds have +25% health and damage.
Rewards: 1 extra ilvl 213 and +1 Emblem of Conquest.
Kite Patchwerk across Frogger, while killing the adds in Grobb's room, then defeat Patchwerk while Grobb is engaged. Now that's a real hardmode.
It might require Ulduar gear to survive/beat the Enrage Timer.

Huh? There was another thread? I searched and couldn't find one. Oh well. Eh. Don't worry about it, the last thread I saw was old and focused only on hard modes for old-world bosses.

Not a hard-mode, just two achievements.
Defeat all the bosses in naxxramas on normal difficulty, with atleast 3 members of the raid going completely nekkid.
Awards the title: "The nekkid" if you where nekkid, and the title: "The voyuerist" if you participated in the event with gear on.

BTW I believe that kiting Patchwerk to Grobb through frogger then killing his trash then Grobb truly would be a hard mode, would be pretty friggin sick IMO.
Its not currently possible (both bosses leash to their room), but another sick boss fight combo would be Heigan + Loatheb - Heigan would be done as normal, with a tank doing the dance with him during phase 1, while Loatheb is tanked and fought on the platform, then when Heigan teleports up, Loatheb has to be tanked while the raid does the dance - all this while the healing aura constantly blocks healing, so if you take damage from the dance, its actually meaningful and can't just be healed through easily, also the spores that would spawn away from Loatheb would have to be quickly killed while you're going back and forth across the room. Overall, I think that combo would be awesome.

Heroic: Kibble Rations
Deafeat Gluth in Naxxramas within 3 minutes, while allowing him to consume at least 10 Zombies.
Let him eat the first 10, then burn him ... don't know off the top of my head what the spawn timer is on zombies though. Sounds doable though.

Hmm, hard modes are a bit tough to come up with, but achievements are doable.

Thaddeus
After defeating Stalagg and Feugen, all but two members of the raid must dive into the slime pit, swim back to the entrance, and then climb the ramp and jump onto the platform before attacking Thaddeus.
The Gang's All Here
Boss:
Gothik the Harvester
The tank and DPS must allow all elite adds to spawn on "live side" before killing any of the elites and sending them over to the "dead" side.

Something I had thought of similar to this would be
Take a Walk on the Live Side
, where the entire raid stays on the live side throughout the encounter.

Play with out any add-ons.
That is all.
that isent a hard mode. you would be suprised how many people do fine with out add ons.

You could just let him eat them as they spawn (not all at once) so you can burst him heaps before he eats the last one, then you dont have his full HP to go through
