It's not from the addon code but there could certainly be an .exe in the zip file that's not part of the addon code. As long as you're getting it from a reputable site, it won't be an issue.
The big issue is people not being aware of how systems can be compromised and how to keep their system up to date and secure. Phishing emails probably get a huge number of people's account info and those are sent daily, multiple times. The rest of the attacks could be from malicious ads on sites or compromises from PDF's, Java, Quicktime, etc.

I suppose it's possible that there's a vulnerability in Flash right now. Some sites like curse.com become channels for Flash-based attacks. What happens is some of the comments on add-on pages have embedded code that does not display; rather, this code is used to exploit a vulnerability in Flash and inject a keylogger or other malicious file.
At least, I'm pretty sure that's how such a vulnerability could be exploited. I'm not 100% on this though, but for your own protection you should update Flash asap. Also, install
Secunia PSI
and let it tell you when you need to update your software.
