I think it's because certain bars are meant for certain forms. For example, on a rogue, bar 7 is also the stealth bar. I'm putting in a request for this to be moved to the UI & Macros forum, since the folks there might have a better understanding of the problem.

So, it happens when you're pressing no buttons, not moving your mouse or clicking anything?

I've seen this with Bartender... and its because I was scolling with a modifier key held down... not even on purpose, just scrolling the camera out while i had key down absent mindedly. It took me a few times to figure out what I had done... unfortunately I did this right before a pull... nothing like casting by clicking on your spell book icons.

Is it that the bars for your forms are showing up as one of the higher bars? Like you pop into Bear Form, drop Maul into slot 3, and it appears in slot 3 on another bar?

Sorry, about the late response.

Whats happening is, when I switch forms, it chooses bar #1 and does not change no matter what form i change into.

i -think- i understand what you're saying, and that's exactly what's supposed to happen.. think of it like you had no UI mods at all, and were using everything exactly as blizzard had it set up. When you shift from caster form to bear form, all your bear form abilities pop up into your main action bar (bar 1), because it's inconvenient and a waste of time for you to switch forms and then have to find the action bar where all your abilities were set up.

Now, with things like Katallian was saying, about having a bear form ability dropped into bar 1 while you're in bear form, and it also pops into bar... 7 for instance, that's just the way it is unfortunatly. Several other bar mods have the same problem. Bars (i think) 7-10 are going to be your stance bars (bear, cat/battle stance/stealth, boomkin/defensive stance, and tree/berzerker stance IIRC) because blizzard only designed so many different action bars in the game overall, and had to have somewhere to stick the different hotkey assignments for the different forms or stances given to different classes.
