First off, some gemming problems.
You should be using almost entirely all +30 stam gems, and never use a Nightmare's tear.
In my kills on him, I would start tanking, so I'd go through his first and second stack, and then his second/third stack on the second go around.
If I noticed the healers struggling a little on the first stack, I would pop a trinket to help out. During the second stack, I would use IBF and once that was over, VB (since by the time I picked him up again, they would both be back up).
Now, on the 2nd and 3rd stack during fester's second rotation, I would use an armor pot/trinkets/AMS for the 2nd stack, and try and save both VB and IBF for the 3rd stack, popping IBF first, and then VB coupled with a healer cd (assuming you have a priest or pally in the raid).

You shouldn't really need to start hitting cooldowns until the 3rd inhale. Before the 1st inhale, the tank damage is relatively light, and after the first spore + inhale is over, the raid healing becomes relatively light as well.
Having played both tank and heals in 10 man festergut, and heals in 25 man, I contest that.
Coming from my tank's pov, considering a dk's cds have a fairly short down time, there's no reason not to use them if you are the first tank, as I stated above.
Coming from my healer's pov, by the second inhale and up until the pungent blight, I'm spamming holy light which uses up mana fairly fast. Thus, any way to mitigate some damage through the 2nd inhale to help with my mana is appreciated.
