What I want is:
When in combat, target nearest enemy... which is easy
/targetenemy
However, when not in combat, I'd like to target any NPC/player.
I tried
/targetenemy
/targetfriend
but this doesn't seem to ever target friendly npcs.

There isn't one that targets friendly, non-player characters only, AFAIK.

Would help if we knew WHY you need it to target friendlies...If its just a matter of not having a hostile target so you can avoid accidentally pulling mobs or something, you could use
/cleartarget
or
/target YourNameHere
to target yourself.

I don't know why it would NEVER target an npc as that's not it's described function, however I wouldn't be shocked to find that the function favors pcs over npcs.
You could try:
/targetenemy
/targetfriend
/targetfriend
/targetfriend
/targetfriend
/targetfriend
/targetfriend
/targetfriend
/targetfriend
Such that if you're in combat it selects an enemy, if you're not it does the normal targetgfriend and (ASSUMING it favors pc over npc) it will seek a new target and change it to an npc if it finds one.

@pelf: Oh, misunderstood the meaning of what you said.
Anyway, it does target npc, but the thing is that it will target nearest target, so most likely, someone of your team.
