Depending on what Totem you are using, I would do Lightning Bolts before ever thinking of casting Chain Lightning Xinnuo. If you are using the triumph badge totem like me, it procs quite often and once proced, I never have a problem with it going down. I only ever cast a Chain Lightning when this procs.

1) Chain Lighting is indeed a single target dps increase AS LONG AS you don't run out of mana during the fight.
--- While it does lower damage per cast (on single target), and only has 11% chance to proc Lightning Overload (per target), the faster cast time makes up the difference. Especially if it's the difference between getting an extra Lightning spell cast or pushing LvB back waiting for a full Lightning Bolt to finish casting.
--- When (re)starting a rotation, it's better to cast LvB, LB and then CL, to ensure Clearcasting is up (saves some mana). While spam cycling your rotation, this isn't as important, because Clearcasting will be up quite a bit, allowing you to get the benefit most of the time. But when restarting the rotation (after movement): LvB, LB, CL, then prioritize as usual. Again, once the rotation is started, the goal is to squeeze as much lightning as you can into the window between LvB _without_ delaying LvB.
--- If you're running out of mana during the fight, and have to Thunderstorm and/or slow down your rotation, you've lost all your dps gains and then some. Read: DPS Loss.
2a) In raids, I use the totem set that the raid leader wants me to drop. Not that (s)he necessarily knows what's best (good ones do, try to run with them), but that it's their job to organize the group. It's your job to do your job.
2b) In a Heroic/Group setting, I drop Stoneskin unless the fight calls for Tremor Totem.
