I haven't played on my DK in a long time and when I did I stuck with frost. Your DPS seems very low for your gear. I could pull that with a mix of t7 level gear and a piece or two from Uld. Check the stickies for rotations. Seems like you have one too many deathcoils.

No you dont. Even if it wasnt severly nerfed and rolling diseases corrected.

Figure out what are the differences yourself. Beside the obvious lack of Reaping.
No sure why you use Death Embrace minor either.
Cheap enchants, pvp enchants, lacking enchants. Lot of low level items. Not leveled proffesions.
Low level gems, cheap (wrong) meta.
Low expertise (3.25% under the cap).
I'd say you are fine at 5k with this crap you got there.
