I forget to use MD *facepalm* but I will remember to use it more often.
Always MD first. Try to make it a habit if possible, even if you're a fair bit away from pulling aggro.
Why?
Two reasons:
1. You can have a lucky string of crits. Getting 10 crits in a row is great for DPS---and bad when your aggro is already near the tank's level.
2. Main reason here: You are raising the threat cap for everyone else. The threat you MD to the tank allows everyone else to push out more damage without pulling aggro. If there is anyone in the raid doing more DPS than you, they will also benefit from your decision to MD often.

In fights with adds, of course, you may want to keep MD available to pull something off a healer if your tank is slow or has his taunts on cooldown (or whatever else can go wrong).
When in doubt, ask your raid leader if you should MD regularly to give everyone some breathing room or save it in case of adds.
In EZ-mode raids like Naxx, it probably doesn't matter. Ulduar is a bit tougher and may actually require some strategy from time to time.
