NO offense intended, but I'm working through your English here. I'll try to answer your questions.
your def SKILL needs to be 540 for raids, 535 for heroics. This means the number you see immediately in your character tab, NOT the number you see when you mouse-over the word "Defense" and make the small window to appear with a larger number. edit: yes 690 is the rating cap, which equals 540 skill
It takes just under 5 defense RATING to equal 1 defense SKILL (4.92 i think, not totally sure)
personally I like to go several points above the 540 cap for added avoidance, and to make it easier to replace gear as it drops.
25k HP is fine for heroics, just be sure your parry+dodge = more than 30%. in fact you should be good to tank in raids, though 28+k is more desirable.
edit: for more def use the Rune of the stoneskin gargoyle, trainable in ebon hold like your other runes, it gives you a solid 25 defense SKILL. if you already have it, you need to focus on def gems and enchantments, but there should be no reason you cant hit the defense cap after using the rune.

25k HP is ok for heroics.
The defense cap is 535 for heroics and 540 for raids. As a death knight you should be using a Two Handed weapon with Rune of the Stoneskin Gargoyle, which gives you +25 defense skill, plus a base of 400 at level 80, yielding 425 Defense, and requiring an additional 115 Defense from gear to become defense capped.
You gain 1 point of Defense from roughly 4.9 points of Defense Rating. I usually estimate on a 5 to 1 ratio to give myself room for error. That means your gear needs about 575 Defense Rating for you to reach the cap.
You can opt to use the Rune of Swordshattering, which gives you +4% parry rating. If you do that, you'll need to get a total of about 700 Defense Rating from your gear. It is usually better to opt for the Stoneskin Gargoyle, because it allows you to focus less on Defense Rating and more Stamina / Parry / Dodge.

I would have to agree with Regoras
I have found that when using Gargoyle, my avoidance AND health have actually both improved after i replaced several sockets and enchants that were for defense with stam and avoidance stats.
overall I am much happier with my stats when using gargoyle after being forced to use sword shattering before.
