I would go Block Rating>Block Value>Dodge>Parry>Def. or maybe Dodge>Block Rating=Value>Parry>Def
Ofcourse, this is the age old mit. vs avoidance discussion...
I know the more def you have the less effective it is...or something like that...
Edit: Eitherway Block/Dodge...parry is ..meh.

While 535/540 are the def cap goals for crit immunity in heroics/raids, I realize that adding defense post-540 is still beneficial for tanking. A few questions on that note:
At what rate does post-540 def add to avoidance and/or mitigation?
1 defense skill = 4.92 defense rating = .04% miss/dodge/parry/block
Before diminishing returns, 41 defense rating = 1% avoidance (miss+dodge+parry) and 31 defense rating = 1% mitigation (miss+dodge+parry+block).
Is it more beneficial to take +parry and +dodge bonuses rather than post-540 def? If so, at what ratios?
39.35 dodge rating = 1% dodge
49.2 parry rating = 1% parry
Speaking very broadly, dodge is slightly superior to defense in pure avoidance. Parry is almost never something you should go out of your way to acquire (about the only situation is when a non-JC needs red gems to activate a meta gem, where parry/stamina is currently the best option). If your goal is a block capped set, defense will be superior to dodge.
What is your hierarchy of stats post-540? (Ex: block>dodge>parry>def ?).
It depends entirely upon what you're attempting to achieve. You gear to the fight. My examples assume you have the needed cushion of health to survive the normal attacks of the boss without too much difficulty and give your healers time to do their job.
Bosses that do heavy magic damage lend themselves to high stamina sets. Good examples would be Kel'thuzad and most dragons.
Fast attacking and/or soft hitting bosses are best dealt with by block capping and block value. Think most raid trash, Loatheb, and MT Patchwerk.
Heavy physical burst is best dealt with through pure avoidance stats (and extra stamina). OT Patchwerk is the obvious example.

I've a question similar to the OP, I'm at 540+def, 22k health and ready to upgrade through heroics. What emphasis should I be putting on getting +Hit, especially if it means losing out on +def or +avoidance.
Your emphasis on +hit just beginning heroics is zero. There are far more important stats for you to worry about at first.

Paladin's don't need hit, even if they miss they generate aggro through classic ways (such as blocking, dodging and parrying) as well as other ways (consecration doesn't miss).
I'd say that after getting 540 defense your nº1 worry is stamina. the more stam the better, no matter what. Block, dodge and parry as well as extra defense, will be added on even if you don't focus on them.
Even so the tree goes like this:
Defense rating>Stamina>Block Rating/Value>Dodge/Parry>Extra defense rating.
again...I'd still do Dodge>Parry

Those are two different actions. To you, it makes no difference, but as far as gaming mechanics go, it's kind of important. WoWWiki had an article about it (somewhere in the Hit mechanics I think), but I didn't bother doing deep reading.

Although defense is very budget-efficient for avoidance and mitigation, if you stack solely defense to pump up avoid+miti, you will be lowering your mana-regen efficiency of Blessing of Sanctuary, which is because you are adding miss chance to your hit table, and push your dodge, parry, block away. Miss benefits nothing to Blessing of Sanctuary.
While this is true, its highly unlikely that you will go oom because to the extent you would have to stack it to make a largely noticeable difference would require an exceptional amount of defence (and i think diminishing returns takes care of any chance of that happening)
