You would have to watch for an event or parse the incoming chat messages. It would not be simple.

If you want it to auto-respond then you'll need an addon, not a macro. If you want a macro then just/g gg would work. I use /g gratz for simple achievements and type in a custom message if I think it's worth it.
Slightly off-topic, but I think auto-gratz addons are very impersonal and can even be rude to some. To get a congratulatory message from someone that you know is afk is so impersonal that I consider it rude. That's just my opinion, not the rule.

i agree (about auto-grats). in my guild we discourage people from saying "grats" at all unless they're actually impressed or know the achiever has put a lot of work in. it doesn't mean anything unless you only say it when it means something.
people don't need congratulating for getting their first tabard, exploring elwynn forest or dinging level 10.
also, the gratser doesn't need to be afk for people to know it was an addon. when someone ALWAYS responds with the same message and always responds instantly (timestamps show it to be too quick for a human to do even if they had a macro ready to press) it'll get annoying for people quick.
