How do you Bears avoid the infamous Magmaw instagib?

Just have everyone run ever so slightly out of melee range as the exposed phase ends leaving only the tank there to pick him up and (hopefully) be the only one getting melee'd.
Edit: You're not actually losing raid members to this are you? He won't even kill a mage in one melee swing unless he crits them.

As far as Mangle goes, it just nom noms the tank and allows Magmaw to be chained down and exposed. Threat on the exposed head is irrelevant since it does nothing but sit there and take damage. As long as the tank is the only person between Magmaw's torso and the impaling spike at the end of P2, no one will be hit but the tank. I'm assuming the people that died were between the spike and Magmaw during the transition from Phase 2 to Phase 1, as that's what it sounds like.
Basically, do what you're doing, but tell your DPS/healers to stay out of Magmaw's melee range, especially during the transition from P2 to P1.

Everyone is cleared. Being a tank you need to Tab out and target the swinging head. DPS it as much as you can and always have Growl ready for you when your down pounding him down. If you have a Hunter or Rogue have them ToT to help your threat generation if you don't have it always Taunt off.
Get Omen or get Skada to see how much threat you need do in order to keep Magmaw on you.
When I first encountered this boss I singled tanked this boss with 145K HP and 3 healers (Holy Priest, Resto Shaman, Holy Paladin)
I dont know how else your healer got killed. My assumption is that he throw a BIG Heal on you right before you were able to generate threat. This is why you need a Addon to help you with threat.

And what I find weird, is the time that the mage died, she told us she was SURE she was out of range, so I'm kind of at a loss for words as I can't explain why she died.
Regardless of whether or not she was sure, she was obviously in range or she wouldn't have died. It's better to run too far from the spike as a dps/healer (why are they that close to his melee range anyway?) than to risk getting 1-shot. The only way people other than the tank take damage are his magma spit (random 25-30k, healer duty) magma spew (raid-wide 25-30k, healer duty) standing in parasite puddles (fail) or parasite kiting fail (fail). The only things that will 1-shot people is a melee crit from them not being where they're sposed to be, standing under the spike when he gets impaled on it, or his raid-wipe ability when no one is in melee range.
Edit: Okay, I just found out about the "everyone but the parasite-kiter standing in the melee dps spot" strat, which would explain why your raiders are too close. Restate the importance of moving out of his melee range when he comes off the spike (which should be significantly easier than moving as a group from the lava pools, which is what that strat is meant to avoid). If they can't handle that, start replacing people. :P

Understand that while the head is exposed, the body in in play but not able to be targeted. This means is the entire time Magmaw is spiked your healers are gaining threat on the body. If one of these healers is within melee range when he surfaces, chances are good (regardless of a growl+mangle+maul - whatever) it's cancel Christmas for your healer.
I hear Magmaw prefers the taste of Paladin healers however he'll eat whoever.

What happens is that he resets aggro completely after his head comes back up from the "point of vulnerability". Everyone should be out of melee range when that happens. The way we do it in my guild is just with one tank. We have two marked positions on the ground for the ranged/healers to run back and forth between on flame pillars. As soon as Magmaw's point of vulnerability ends (if you run DBM there will be a timer for it), and his head comes back off the ground, target, mangle, and taunt. Everyone else (even the melee) should run towards one of the "safe points". The melee, obviously, don't have to stay there. Just run towards it, then turn and run back once your threat is back on top.
Also, as a side note that I didn't realize for a bit, when you're being mangled, you can tab and target the exposed head of magmaw, even before he drops you back down.

Yes, great tip. This is something I tell the other feral tanks in my guild. You can help your healer by keeping those SD bubbles up during mangle.

This also applies for blood shield on DK tanks... and it's a little extra dps in case that's an issue. /shrug
