Old Gods? Even without realizing it?
The Old Gods convinced Deathwing that he was supposed to rule over all the dragonflights and Azeroth. He wasn't working for them, just for him. Obviously, through his actions the Old Gods wanted to free themselves.

erm, he was driven mad by the Old Gods and is currently kind of in charge of twilight hammer. Yeah... what's your point? Deathwing more or less *!@#ed in their giant faces during War of the Ancients when he tried to steal back the Demon Soul as it was being used to open a portal for Sargeras, a portal they were intending to steal and use to get free of their prisons. As for the Twilight Hammer; the Hammer serves anyone who is trying to bring about the end of the World, and considering what Deathwing does when he rips free of the Elemental Plane, dragging Deepholme up and weakening the gates to the other Realms, I'd say the Hammer see potential in the Destroyer.

The Twilight's Hammer is going to play a major role in in the Cataclysm expansion, as they now serve Deathwing, and they are the ones responsible for bringing him back to full health in Temple of Earth. They will make a strong presence in Deepholm and Twilight Highlands areas of Azeroth, and will also appear in places such as Mount Hyjal (mainly Darkwhisper Gorge) and even Uldum. This is an ironic reversal as they were previously allied with Dark Iron Dwarves, fierce opponents of Nefarian, Deathwing's son back in Blackrock Mountain.

The Twilight Hammer cultists might not know that the Destroyer was the responsible (in part) of preventing their "Gods" for breaking free 10.000 years ago.

I don't know. maybe they weren't established as the cult yet, or they numbers were low. During the time between W3:RoC and WoW they more people joined their cause.

Part of the Dark Horde for the Second War; then they vanished and started raising up converts over the course from the end of the Second War to the start of WoW; see that as seeing them in Silithus, Darkshore, or any other place with a connection to either an Old God or Elemental Lord(aka the Twilight Hammer Cultists in Blackrock Depths).
