From an experience prospective youll want to do ICC10n as well, yes, thats why I stated from a gear relevance, TOC10h is enough for ICC10h.
I replaced my 213 cloak with the 264 one. :D and Id still have the black heart on if not for brewfest so youre pretty much right on there.

Luckily you can't get into ICC10 hard without having done ICC10 normal (pretty much ad nauseam(sp?)). But skill/experience > gear. It might be possible to do it but you'll need to be bloody awesome :P

I don't want to sound silly but 10-man and 25-man intentionally have seperate progession paths. That means that you could theoretically do just 10-mans and progress in them without having to do 25-mans to gear up.
Having that said, let's have a look at item levels and where they drop (safe from hard modes, etc):
Naxxramas 10: 200, 25: 213
Ulduar 10: 219, 25: 226
TOC 10: 232, 25: 245
ICC 10: 251, 25: 264
So to tank ICC10, you basically have to have stuff you could collect from TOC10. That means item level 232 items and perhaps crafted 245 items and the ones from emblems. There is no need to have an average item level of 245 to do ICC10.
The same goes for the 25-man. If you want to start ICC25, you will need 245 items and not 251 (which drop in ICC10). Otherwise a 25-man raid guild would be forced to collect gear in ICC10 first before attempting ICC25.
In fact my Paladin has only heroics/emblem gear (232, with some 245 and some 219, and even 2 213 pieces) and can tank the first few bosses in ICC25 without any problem.

Funny thing is there is people out there who would laugh at and kick out said tank because of his low health. A resto driud real life friend of mine was making fun of a DK tank with 28k in a heroic. All I could do was /facepalm and tell him hes lucky he didn't heal heroics when WOTLK first released and a tank in 20k health was perfect for heroics.
Sunwell raid gear, quest blues, and item level 200 crafted epics FTW.
