I'm going to point out a couple things:
Most healers, especially now with the new queuing system, would rather be dpsing. So be grateful that they did take it upon themselves to heal for you.
The healer is not obligated to make up for the dps' mistakes. Most dps classes have an aggro wipe or some sort of defensive cooldown. Use it.
The healer and the tank are the backbone of the group; dps are expendable. Remember that.
There are people behind those characters. Never demand anything. Ask. Respect.
Take the first step. A healer will almost always prioritize healing a dps who goes out of his way to protect the healer.
Whatever you do,
do not ever say 'heal me'
. Most healers watch the chat box out of the corner of their eye, and anything you type will cause them to look away from your health bar.
Thank the healer. Even if you know you'll probably never run with that healer again, one little sign of gratitude can go a long way.

Tanks are the top priority, but I hate healers that don't heal DPS when the tank is at/near full. It's just lazy.
That said, speaking as a healer, if you're a hunter, a warlock, or a DK, you can heal your own goddamn pet. My Grid doesn't even show them, and I assure that is intentional.

This is important. I've met a few healers who refused to heal self inflicted damage and then got all ^&*!@y about it.
E.g. HoL, that lightning thingy that damages you while moving. Low damage, easy to heal through, when dpsing I usually just ignore it so I can run to the next mob and deal damage, making the run faster. A healer actually let me die (well, I got to very low HP and when I noticed I stopped running ... but some random AoE killed me afterwards since I was standing close to mobs) because of that on purpose. After I told the healer how stupid this behavior is she refused to heal me for the rest of the instance. Which resulted in my just running behind the group, not risking to get any damage and not dealing any kind of significant damage and slowing down the run a lot.
Why would you intentionally take more damage? do you like making the healers job harder? it's not even like you gain any extra dps by doing it either. Intentional damage is only allowed IMO, if it somehow helps the raid (locks life tapping in combat). you sir, are either extremely stupid.

You don't want to heal my pet? Okay, fine - I can accept that. But do be aware that you're risking a wipe and your own death, as a result of that decision.
I think I'd rather see the healer healing the tank before any pet. If a pet dies there MAY be a risk of a wipe (though I highly doubt it). If the tank dies, a wipe is a certainty.
I've played a lot of classes. I've tanked with my warrior, healed with my paladin and priest and done DPS with my hunter, warlock and mage. I don't EXPECT my pets to get healed. I don't even expect my warlock to be healed if I'm constantly life tapping. I realize that there is SO MUCH a healer can do and unless I'm the tank I'm lower on the list and that is especially so with my pet.
You know what I see a lot with pets? I see compete wipes due to the pet not being controlled properly. When I played my hunter and warlock I'd put my pet on passive or completely dismiss it. As someone else pointed out, unless you are a Beastmaster Hunter or Demonology Warlock, the pet isn't doing much damage to make that big a difference and is more often a problem then a help.
That said, as a healer I have to prioritize my time and mana. Paladins tend to be single target healers. Yes, we have Beacon of Light and a talent that gives us a small HoT on ONE person in conjunction with Sacred Shield so that means we have to be careful about prioritizing. As a result, that means the Tank gets the bulk of my heals, followed by myself and then comes the DPS. I will spare a heal to a pet if I can but it's not a priority.
As far as my healer doing any DPS and or/CC and "helping" out, IMO that's a huge NO-NO unless I'm
specifically
told do something by the raid leader. Otherwise I do my job, which is to heal.

Can you imagine what the grid would look like if you were to include pets in a 25 man raid? The horror! It must have really sucked to heal those old 40 man raids...
Obviously, it would be ridiculous to heal pets in a raid. Especially, if you're the Tank healer. I'm not sure about everyone else, but I'm referring to 5-man instances.

I'm not sure about DK's or Hunter, but a Warlock's
Health Funnel
takes a full 10 seconds to channel. Now that's 10 full seconds of dps wasted because you couldn't throw one HoT on their felguard, even though everyone's at full health, because you're too lazy. I would understand if you could not get to the pet because a player is taking top priority, though.
I see that there are few opportunities where you
can
heal the pet, but when they arise, please don't be stingy.

Hyper, I completely agree with your post.
I'll assume you're talking about good healers, though. Obviously, healers can make mistakes too. And not every healer in the world is going to be polite, understanding, good at their job, etc. But, I'll most certainly agree that many people do treat their healers inappropriately.

What's to understand? It's the Hunter's responsibility. There's a reason that you have Mend Pet. I'm a priest, not a vet. If we're in any kind of situation where a pet is in danger of dying, I've got way better things to do than keep an eye on your goddamn turtle. If I have time to search around for the little guy and check his health, then we're facerolling a heroic and I still can't be bothered to heal your turtle.
In conclusion, F&*% your turtle. :D

1. Heal the pet.I think this depends on the healer's class and the situation. Beacon of Light is wonderful for this - doesn't matter what you're spamming on, the tank still gets heals (overhealing transfers). If no one else it taking damage, I get to watch a light show on the water elemental standing next to me. Now, there is a priority of course, and as with every healer, it should be:
1. Tank
2. You
3. Dps
4. Pets
So Fuzzy is only going to be getting attention from me if everyone else is doing fine.
2. Heal the DPS. A healer who only heals the tank (unless that's specifically their job, such as in a 25-man) is flat out doing it wrong. You don't have to keep everyone at 100% (that's an easy way to go OoM), but letting party/raid members die while you prance about spamming Lifebloom on the tank is a quick way to get a swift kick (in the junk, if I had my way).
3. Help yourself and the group. Essentially what you're saying here is to remember that you have much more in your arsenal than heal and cleanse. Those two things take priority, but things like Hammer of Justice, Hurricane (yes it pops tree form, but you're overgeared, right?), Hex (CC? No wai!) and Dispel Magic (on the enemy as well as your friends), used in the right situations, can keep the pull in control and speed up your pace.
And for heaven's sake just finish occulus, it's really not that hard.

Mend pet is a very weak HoT spell. Usually if the pet is taking enough damage to warrant healing, Mend Pet is not going to save it. It heals for too little over 15 seconds. I bandage my pet if he needs healing in pugs because usually the group are in too much of a hurry to wait for Mend Pet.
I heal pets on my healer if I can. I don't ask healers to heal my pet on my hunter though unless it is a guild run and I know the healer and know they can do it (of course I usually don't have to in that case).
