Because it cant be insta-cast with a 5MW stack. If youre stopping to cast this spell, youre losing out on possible DPS, much more than what you would gain by casting it. Thats the easy answer, but Im sure you can find the number crunchers on here that can give a much more detailed and extensive answer.

whilst casting LvB you cannot gain Flurry, 10% extra AP, proc most trinkets, have Windfury proc. This and it resets your swing timer.

It still has it's uses if you have nothing to do when you're not allowed to get near the boss.

This is the only time the spell should ever be cast as Enhancement.
An example of this would be after you've gotten clear of the Shock Blast on Mimiron, while he's still casting it (and if you're stupid enough to say "that means you moved out of Shock Blast too early", nobody's connection is perfect, if you aren't moving out ASAP, you're a bad player).

It used to be attatched to MW but they removed that sometime in beta. Good if you ask me, we'd probably use SP gear if it was still viable.

LvB does quite a bit of damage. Now that FS is viable for enhancement it might be worth it resetting the swing timer. Assuming you can time it right you will probably use only one white swing from each weapon, right? Adding up that damage plus the WF proc chance (or better even: time LvB so that it's only used when WF is on ICD) might really yield some extra damage.
I wouldn't dismiss the idea right away, at least not before I see some numbers.

If you're casting LvB you're stalling the use of your other abilities, preventing MW and LS from proccing. The cast is far too long for you to not clip your swing timer also.
As for putting LvB into MW? Aside from the fact that this may push us into stacking SP, I think we have enough things to watch for already.

Ha, why are you being so defensive. Made me laugh, are you being ironic?
The last time I suggested lava bursting once you're clear of the Shock Blast, like five guys yelled at me that I was "doing it wrong" by playing it safe and getting clear early instead of living on the edge and getting out JUST before the explosion, to get those couple extra hits. I was just covering my bases :D

well in theory your doing it wrong by casting instead of meleeing, but the reason you might see an increase is because of how wierd enhancement dps can be, you could be sitting at 5k dps on minute and the next at 3k if you were unlucky, blizzard said that lavaburst would be very OP, but the mechanics for maelstrom weapon are decent at thier best (when heroism/ bloodlust is up) I personly want to see maelstrom weapon giving us a free lavaburst and having stormstrike give a bonus to spells besides flametounge weapon (since it would eat the stack) so that enhancement dps is much more stable
my wall of text crits you for 9001
