Mary-Sue's Origins

I've seen it time and time again on these boards: Characters being referred to as "Mary-Sue." Now, I understand the basic meaning of it; that whatever character is 'too' good and seems to lack flaws, thus making them seem "out-of-realism" but where the heck did this saying come from? Who was the original Mary-Sue?
