As soon as you're on the mob's threat list, you should see it pop up. If you haven't thrown a heal yet, you might have to have the mob targeted.
Do you use healbot? That's how I check threat. When someone gets up there and pulls, it highlights their whole box (or you can set other ways to show threat pulling, like red border, etc.)

Also, remember that Omen will (by default?) only show threat on your target.

Maybe that's it because I usually don't target mobs unless I have lots of excess mana to also throw dots.
I'll also have to check my healbot if I see some threat indicator there.
To elaborate a bit, the threat API doesn't require you to be targeting the mob to be able to read threat data. All the API requires is a valid UnitId. So, if there's a massive trash pull of spiders in an instance and nobody is targeting spider #32, the threat levels on it can't be determined. However, if the hunter in group 3's pet is targeting it, then that's (maybe)
raid12pettarget
, a valid UnitId. If spider #32 goes to heal spider #20, then there's a valid UnitId for that, now, too:
raid12pettargettarget
.
Anyhow. Omen shows threat on your target. Anything can show threat on anything that's a valid UnitId.
