1) Throw a judgement or a crusader strike on the mob. Its fairly easy for you to do more threat than a healer does. If the tank is paying attention and decently competent, he'll have no trouble picking it up off of you. Alternately, Hand of Protection is your friend, as is Hammer of Justice.
2) Avenging Wrath is fine. Just be sure to give your tank a threat lead before you pop it.

Download a threat meter (like omen) so you can easily tell how you are on threat. And use avenging wrath every time you can without pulling too much threat.
Threat should rarely be an issue unless your tank doesn't know what he's doing or you out gear him.

Your most important objective in any fight is to stay alive. Some people focus too much on dps'ing that they forget that you do 0 dps while you're dead.
Your secondary objective is, of course, to hit things till they stop moving. (Avenging Wrath should be fine, as long as your tank is relatively competent and you've let them at least touch the mob you're attacking)
Once you've completed the above two things then you can try to help out others by tossing a flash here and there, Cleansing, Hand of Protection, etc.

You need to gauge your tank's threat within the first few pulls. If he's a stellar tank then you can go buck, and use all your abilities. If they are lacking in AoE threat, might want to ease up a bit. Generally though if the tank is a DK, I usually lay down a consecrate right above their Death and Decay.
As far as what you've said about protecting the healer, I do it all the time. Either when I play my warrior as Fury, Ret Paladin, or even on my wife's mage; I go after the mob that got away. As a Ret Paladin you should have the tools to be able to distract the extra mob for a bit. Either HoJ or Repentance should work to save them, even allowing the mob to hit you a bit. Chances are you'll last longer than the healer will. I use to slow the extra mob on my wife's mage and either frost nova or sheep em. You can't help having that situational awareness as a tank and it could mean saving your group from a wipe.
