IceHUD because it's ridiculously customizable
I used IceHUD for ages. I loved the fact that its castbar accounted for lag, which allowed me to have almost instantaneous spell haste.
However, I assume that by "customisable" you mean in terms of aesthetics only, because you had no choice over what info was displayed.
I'm really liking Carbonite and Konnektiv Unit Frames atm. Konnektiv are nicer looking and a lot more lightweight than X-Perl, without skimping on info like a lot of other lightweight Unit Frame mods.
I don't know if Carbonite is really that unknown (though I'd never heard of it until recently), but it's 1000 times better than Questhelper, plus it gives you a combat timer, dps meter, minimap, minimap buttons, a comprehensive bag inventory, plus all the info that you would otherwise get from Titan and Bonus Scanner

For my Rogues and Druids, I like to use
UIM:ComboPoints
. A bit out of date, but still works great! I also use
CustomChat
on all my characters.
I'd be kind of surprised if these two are well known. 8>)

Here is the list of not well known addons I use:
- Combuctor (bags)
- Blacklist (49 is not enough ignore)
- NoteBook (an ingame notebook)
- Peggle (Peggle is awesome, into wow it is awesomer :D)
- WildDPS (Personal DPS/Healing/Agro meter I made. It is very accurate, but is unfinished and I dont have time to work more on it).
- OpenAll
For my hunter I have 2 extra:
- DavAspectFrame (It is outdated but I modified it to add dragonhawk and viper aspect)
- DavTrapFrame

one of the BEST addons I have for my mage is magealert. It lets you know when you get a MB/HS/BF proc with a flash and an alert sound.
I also use an addon called ackis recipe list. It shows any recipe's you don't have for any tradeskill as well as how you can obtain it.

my choice is rActionBarStyler and rActionButtonStyler.
these are meant to be subtitutes of bartender/dominos and buttonfacade, using near 0 memory. they dont have so many options to customize, but they do the work perfectly.
