Drust Stele: The Circle, I found by accident. I was at Gol Osirg in Drustvar and got swept off the waterfall, and landed on a spot with the Cairn.

Here is a list of all Drust Stele locations:

Drust Stele: Conflict: About halfway up the path between Fallhaven and Arom's Stand. Near where the Yeti's are, it is on the southeast side of a curve in the road, south of a small pond. It is behind some other ruins. Hopefully that helps, I don't have any coords addons.

Drust Stele: Protectors of the Forest: In Ulfar's Den, at the back behind the ruins.

Drust Stele: The Tree: At the bottom of a waterfall near the pond/lake at the northern edge of the Crimson Forest.

Drust Stele: The Circle: At the bottom of a waterfall after getting swept off the cliff at Gol Osigr.

Drust Stele: Breath Into Stone: At Gol Koval, behind some of the ruins closest to the ice cave.

Drust Stele: Constructs: At the bottom of the waterfall of the river running through Fallhaven, behind a wall.

Drust Stele: Sacrifice: At the bottom of the waterfall closest to the large tree in the Crimson Forest.

Drust Stele: The Cycle: Just outside of and to the right slightly of Deathcap's cave.

Drust Stele: The Ritual: Can be found smack dab in the middle of Iceveil Glacier, east of Gol Koval, behind a giant mound of ice.

Drust Stele: The Flayed Man: Can be found on the mountain south of Gol Koval, requires lots of jumping and gliding to get to. At /way 56.5, 86.

Drust Stele: The Circle: South of Arom's Stand in the Gol Osigr ruins. You have to jump down the waterfall coming out of the ice, stele is near the rock.

Drust Stele: The Tree: North of Crimson Forest underwater near the waterfall. There is a bridge nearby.
Drust Stele: Sacrifice: North of the giant tree in Crimson Forest. There are few waterfalls falling into a pond, stele is underwater near the waterfall.
Drust Stele: The Cycle: In the west part of Crimson forest near the wall. There is a cave with Deathcap rare nearby .
Drust Stele: The Flayed Man: Up the mountain south of Gol Koval. You don't have to use glider! Path up the mountain starts east of Gol Koval giant cave.
Go up the east side of the mountain almost to the top then jump down on the stele stone.
But be carefull because you can kill yourself if you miss.

Drust Stele: The Ritual: In the mountains south-west of Gol Koval ruins. Path up the mountain starts east of the giant cave.
Take the path and go in direction of Crimson Forest, you'll come up to the hill, stele is rught under the hill hidden in a tree.
Drust Stele: Breath Into Stone: In the north part of Gol Koval near the ruins wall.
Drust Stele: Constructs: Near the waterfall south-west of Fallhaven. It's behind the wall.
Just follow the river to the waterfall and you'll come up to it.
Drust Stele: Protectors of the Forest: In the Ulfar's Den north-east of Arom's Stand behind the wall. You can find Ulfar's Den if you go north from Arom's Stand along the east mountain. Eventually you'll come to a path to the other side of the mountain where the den is.

This might be obvious to most people, but it wasn't to me. You can loot the brews stealthed as a cat but you CANNOT be turned into a newt. I had to collect the 4 then switch out of cat form to drink the last bottle to net the achievement!

Drust Stele: Protectors of the Forest
<A great bramblebear, possibly a Drust druid, is posed with their head bowed to a massive stag and doe. The deer appear to be blessing the bear or accepting its supplication. The stag's coat was once whitewashed while the doe's was covered with blackened soot.>

Drust Stele: Constructs
<There are many scenes on this stone but three are clear. The Drust are building large, person-shaped constructs in one image. In a second, a peson is killed over the chest of one construct. In a third the stone creature walks into battle, taller than even the Drust.>

Drust Stele: The Circle
<The weathered cairn depicts a group of Drust in a circle. A horned creature, possibly a stag, possibly something else, is being sacrificed.>

Drust Stele: The Cycle
<This image shows a single tree in multiple states of life. On the right the leaves are small and low to the ground, like a sapling. In the center of the tree the brances are tall and strong, the leaves large and open. On the left the brances are thin, broken and mostly bare. The last vestiges of dried red-black pigment pling to the ancient stone to mark fallen leaves. A stylized sun passes over the tree.>

Drust Stele: Sacrifice
<The weathered stone has a worn groove across the middle, as if a blade has struck down the same line over centuries. Dark stains mark one side of the worn cut.>

Drust Stele: The Ritual
<The Drust carved onto this stone appears to carry a curved blade in one hand. The head of a man or animal is in his other hand. At his feet are the bodies of many slain animals. Their entrails are being observed by other Drust.>

Drust Stele: Breath Into Stone
<The carving looks like breath or wind is being drawn from a body and put into a stone.>

Drust Stele: Conflict
<This stone shows multiple conflicts or one great battle with many scenes. In the oldest they fight beings that look like themselves or great beasts. In others they fight much smaller beings that resemble gnomes in stature. In another the Drust are driving some Naga back into the seas. Where the carving is most recent the Drust are fighting men and women bearing anchor sigils.>

Drust Stele: The Tree
<The carving shows a massive tree, bigger than the others marked onto the stone. It is surrounded by worshipful Drust.>

Drust Stele: The Flayed Man
<The carving on this stone is a life-sized Drust who has been flayed. The placement of important organs is surprisingly accurate despite the stylization and primitive materials. The eyes are white stones with the faint remains of paint on them. They seem to follow you around. At one point there were prayers, or instructions, carved around the body, but time has rendered the runes illegible. The carving was done with great care, but the purpose remains unknown.>

Looks like Drust Stele: Protectors of the Forest is not visible during The Old Bear. But it is visible during the three follow-up quests.

Just a minor item of interest this achievement name is a nod to the catchphrase, "Just the facts, Ma'am" of Sergeant Joe Friday, the lead character of the 1950s American police procedural TV drama series Dragnet.

I stumbled into The Flayed Man as an outlaw rogue who was exploring the mountain, thinking that there just had to be a stele in the area. I did not use a glider; I used my trusty grappling hook!

None are particularly difficult with flying available, especially on a max level character. The 2 underwater ones were a little tricky, but the videos below show you the way. No problems with this one. :)
FYI: The Steles are clickable stones, similar to the tidesages scroll clicking achievement.
