This item can be re-aquired from Wavespeaker Valoren if you talk to her AFTER all 3 visions are completed if you need to revisit them to complete any skipped quest. Below is a primary example of why you may need this item:

IF you had NOT completed this quest after doing all the 3 visions and turned them in you can get the Blade item from the original NPC in the cave (Tranquil Wash) Wavespeaker Valoren <The Earthen Ring> . She will have an option to "Revisit" the vision. Select it and you will be able to revisit ANY dream/vision.

Now here is a few things you should know:
1) IF YOU HAVE the quest in your log and go to the second vision location and USE the blade and try to swim to the area the quest is located, YOU WILL BE teleported back to the second location and WILL realize your doomed to NOT complete this quest. HOWEVER indeed your not. YOU MUST ABANDON the quest and USE the Blade in that 2nd vision location and swim there in Naga form and omfg guess what.... that's right your not being teleported back to the vision location. You can now REACCEPT the quest and complete it.

2) YOU MUST Abandon quest to be able to swim to the other zone in Naga form.

3) The Zone NPC for the quest is 57/85

4) The Blade Vision location is 29/78

5) The NPC to retrieve the Blade AFTER all visions were completed is: 49/57

6) IF you need to redo any quest the other 2 remaining vision zone coordinates is:
40/75 (First)
29/78 (Second)
33/77 (3rd) - This is up the stairs/ramp in a room and on 2nd Floor directly above the room entrance

If you got lost doing Visions of the Past: The Slaughter of Biel'aran Ridge you can drop the quest and retrieve the Blade from Wavespeaker Valoren <The Earthen Ring> at 39,78 on the middle tier. Your vision will be at 29,79.

I seem to have dropped this blade, I went back to the cave (horde btw) but the lady was not there. I have been swimming all over vashir and cant find anywhere to get this back. Anyone know where you can get the bald back if your horde and havent completed all the visions?

Okay...for those of you who have completely abandoned the quest and cannot find Wavespeaker Valoren, Ruins of Vashj'ir (40, 75) are the coordinates. Remember you are in an ocean and therefore you can go up or down...go up in this case. This is where the original quest was given. The location is called Nar'shola Terrace.

I have completed the zone quest achievement but I'm going back and am using an addon called Questhubber, which shows this to be the only quest in all of Vashj'ir I have not completed.

I no longer have the blade of course, and checked all 4 locations logged for Wavespeaker Valoren with no luck. The only place I've found Wavespeaker Valoren is inside Nespirah, near the upper back level, lamenting over all the corpses around her. She doesn't offer the blade.

Not sure if it's simply not possible, but she's not at Tranquil Wash or on any tiers in Ruins of Vashj'ir :'(.

Bummer.

Yeah the BattleMaiden "transform" spot is indeed at 29,78.5 but also on the 2nd "tier" not at "ground" level, also you need to swim NNE a bit then swim slightly to the S and find the bridge to cross (it's broken.... but otherwise ya swim out of the "vision of the past" area and get kicked back to 29, 78.5.

Those w/o coords.... SW "corner" second level, near the naga "ghosts" near the dome area in ruins of Vash'jir. And g/l and get a simple coordinate add-on ;)
